- April 7, 1965 - While in cruise, the right hand propeller blade failed 30 inches from the hub and seperated, striking the left engine cowl and propeller. The aircraft was forced to land off of the Jacksonville, TX airport, where it collided with trees. (NTSB Report FTW66F0207)

- Reduced to spare parts - Examination of the wreckage shows the port wing sheared mid-span, and the starboard nacelle crushed all the way back to the front wing spar. Evidently from impacting a tree.
